We accept most laboratory orders accompanied by a requisition form from any ordering physician. This ensures that specimen collection can proceed efficiently, regardless of the specific requisition form used.

If you need labs drawn for your ordering physician and we are not in network with the insurance company above, there would be a $25 lab service fee to draw, process and send it to the lab.
